Rationale –

S.L. 2015-46 which revises G.S. 115C-105.41, removes the State legal requirement for developing a Personal Education Plan (PEP) for individual students. While making this change, the revised legislation does require that “’transition plans’ are developed for students at risk that assist with successful transition between grade spans.”

Guidance from the North Carolina Department of Public Instruction “requires the identification of students at risk of academic failure. Best practice supports the need for a method to document the academic profile of students receiving interventions in a more granular level than a cumulative record provides. Capturing a student’s response to research-based interventions is an essential component to problem-solving identified student concerns. Each school, LEA, and state operated program should determine the extent and format of this documentation. Best practice also stresses that parents/guardians should be an intricate part of the problem-solving process when student concerns are identified. Guidance on parent involvement should be developed at the local level.”

Given this, the Onslow County School System will use the Multi-Tiered System of Support (MTSS) Team Initiated Problem Solving (TIPS) Model for identification of at-risk students and development of transition plans.

Student Success Transition Plans –

All students will receive Tier 1, or Core, instruction. This is identified as evidence-based practices and programs demonstrated to produce good academic and behavioral outcomes for the majority of students.

Groups of students needing assistance beyond the Core will have interventions developed by the Problem Solving Team using the MTSS TIPS process. These Tier 2, or Supplementary, interventions will be research-based and utilize a team approach, built on collaboration, precise problem-solving and data analysis. These interventions will be implemented and monitored for effectiveness.

Individual students who do not respond to the Supplemental supports will be considered to be “at-risk” and be provided interventions at the Tier 3, or Intensive, level. At this level, the Problem Solving Team will develop interventions using the Onslow County Schools – Student Success Transition Plan (SSTP). The SSTP will require parental input and signature, and will be attached to the student’s PowerSchool profile. These interventions will be implemented and monitored for effectiveness.

If the Problem Solving Team determines that a student’s deficit is not the result of a lack of appropriate Core and Supplemental support in ELA, Math or LEP, and the student is responding at a rate insufficient to reduce risk over time, the student may be referred for Exceptional Children’s Assessment.

Student Success Grade Span Transition Plans –

When students identified as “at-risk,” and who are receiving interventions at the Tier 3 level move from elementary to middle school, or from middle to high school, the Problem Solving Team at the “sending school” will prepare a transition plan to assist with successful transitions between these grade spans.

This Onslow County Schools – Student Success Grade Span Transition Plan will include the student’s current academic information, academic/behavioral strengths and areas of concern, and scheduling considerations for the receiving school.

The Student Success Grade Span Transition Plan will be provided to the “receiving school” as part of the cumulative records transition process.
