A Medical Release Form and the ALTITUDE Permission Form can be found in the File Library. If you must have medication with you the night of Project Graduation, the Medical Release Form must be filled out and turned in to Ms. Kellum PRIOR to that night.
ALL those attending Project Graduation MUST complete and return the ALTITUDE Permission Form prior to the event to be able to participate in that event.
