This form is your agreement with all policies, procedures and other elements related to the Marching Band.
This form also serves as their FIELD TRIP PERMISSION FORM for all traveling activies.

2023 Medical Form
While a physical is not required, we still need to keep information on file in case of a medical emergency. This will allow us to make sure the student receives the proper care as needed.
Example: If we order food for band students, we need to know if they have allergies (including gluten)
