Welcome to the Onslow County Schools IT Forms section! This page provides essential resources for staff to request technology support, including software, communication services, website management, and general assistance across our 43 schools. Forms are available on this page, and for submission instructions or assistance, contact your site IT coordinator.
Software Requests

Software and Web Application Request Form: Use the Software and Web Application Request Form to request software or web-based programs. Provide your first and last name, school, and the name of the software/website. Specify if it’s for software installation or website access. Upon submission, approval is sent to your school Principal, with a notification to your school DLTF.
Communication Services

OCS Account and Tier 1 Request: Use the OCS Account and Tier 1 Request Form to manage accounts. Provide school name, authorized requester, and user status (Create, Delete, or Transfer). Specify needs (Windows login, email, or Tier 1 laptop for teachers), employee details (payroll name, 10-digit UID from NCEdCloud/HRMS, position), transfer specifics (From/To schools), and start/end date.

Extension and Voicemail Request: Use the Extension and Voicemail Request form to set up new user voicemail or update phone extensions. Provide previous and new extension numbers, previous and new names, and indicate if voicemail setup is needed (Yes/No). For phone issues, submit a Work Order with your assigned IT Technician (Planner).

Phone Move Request: Use the Phone Move Request Form to request IT to move a phone within your school. Specify school/site (from 42 options), current location, new location, data drop availability (Yes/No/I Don’t Know), and reason for the move.
Website Management

Block/Unblock Websites Request: Use the Application for Website Access/Block form to request website blocking or allowance. Provide school name, authorizing administrator, website name and URL, block/allow preference, access for staff/students/both, desired date, and explanation of need.
External Course Access

NCVPS/CCCC Course Access Request: Use the NCVPS/CCCC Course Access Request Form to request temporary access to blocked sites for students enrolled in external programs like NCVPS or CCCC. This form is specifically for students whose coursework requires access to resources outside our district's control, such as Google Drive. Provide the student's name, school, course name, specific website(s) or resource(s) needed and duration of access required. Please note this form is not for general access requests or in-district teaching needs.
General Support

Staff IT Work Order: Use the IT Work Order form to report device malfunctions. Select your location (from school options) and device type (e.g., Computer, Printer, Interactive Board). For computers, specify the issue (e.g., no internet, software needed, physical damage, other), provide the service tag and additional info, and enter your room number. Questions vary by device type; contact your site IT coordinator for guidance.
