This form is required for all absences.  Form is also available at the Front Office and the Guidance Office.  Please pay close attention to information in RED.

 NOTE:  When a student must miss school, a written note of explanation, along with an Attendance Form for the absence, must be signed by a parent and or legal guardian. Attendance form must be presented within 5 school days after absence.  After the 5 day period, attendance notes will be filed but considered as unlawful absence/unexcused.
