This contract is the contract for ALL SBHS Guitar students to fill out.
It is a MS Form that we will do in class when you receive your guitar.(or on your own if you have a replacement guitar.) 

THIS MUST BE COMPLETED, FILLED AND SUBMITTED TO BE ABLE TO TAKE GUITARS HOME FOR THE SEMESTER
